First pediatric visit prep checklist

The first pediatric visit moves quickly. A little prep turns it into answers instead of forgotten questions. This list is logistics — not medical advice. Follow any timing your hospital or pediatrician already gave you for the first weight check.

Checklist

Before you leave home

• Confirm appointment time, address, parking, and stroller/car-seat policy for the lobby
• Pack diaper bag: diapers, wipes, cream, burp cloth, spare onesie, feeding supplies you use
• Bring a light blanket or cover you already use (skip brand-new gear)
• Charge your phone; bring a short written question list as backup
• Note last feed time and last wet/dirty diaper before walking out
• If using a car seat: allow extra time for install and building entry
• Plan a buffer for a feed in the parking lot if the baby is hungry on arrival

Paperwork and admin

• Photo ID for the accompanying adult
• Insurance card and any newborn ID/hospital paperwork you were given
• Hospital discharge summary / newborn record if provided
• Pediatric practice new-patient forms completed online if available
• List of parental medications only if relevant to household safety questions — keep it short
• Emergency contacts and preferred pharmacy name
• Know how the newborn was (or will be) added to insurance; ask the front desk what they need

What to track for the clinician

• Rough feed log for the past 24–48 hours (times; breast and/or approximate volumes)
• Wet diaper count and stool pattern as best you can
• Birth weight and discharge weight if you have them
• Any jaundice concerns already flagged by hospital staff
• Breathing, temperature-taking method you use at home, and any fever guidance you were given
• Redness, discharge, or healing notes for cord or circumcision if applicable
• Feeding pain for the nursing parent (location/timing) if relevant — clinical follow-up may be needed

Questions worth asking

• When should we return for the next weight check?
• Which symptoms mean call today vs. after-hours vs. ER for this age?
• Preferred after-hours number and portal norms
• Vitamin D / feeding supplements: what does this practice recommend for our plan? (confirm dosing with them — do not copy the internet)
• Circumcision or cord care specifics for our situation
• How to reach lactation help if we need it
• Vaccine schedule overview for the months ahead (high level)

In the visit

• Undress baby when asked; keep a warm layer ready after exam
• Write down weight and any instructions before you leave the room
• Confirm how results or referrals will be communicated
• Schedule the next appointment before exiting if possible
• Ask front desk about bill estimates / what insurance was billed if you need a range

After you get home

• Transfer instructions into your shared household note
• Store discharge papers and visit summary in the baby admin folder
• Fill any prescribed items the same day if clinically directed
• Message the portal only for non-urgent clarifications; call for urgent concerns
• Update your “when to call” note with anything age-specific they told you
