Newborn first-month paperwork checklist

The first month produces a surprising amount of paperwork. Do it in short blocks between feeds. State processes differ; use your hospital discharge packet and your state's vital records site as the source of truth. This is not legal or tax advice.

Checklist

Before you leave the hospital (or within a few days)

• Confirm birth certificate worksheet was submitted; keep your copy
• Confirm Social Security number application was started if offered in-hospital
• Get written discharge summary / newborn record for the pediatrician
• Photograph or scan: wristbands, discharge meds list, feeding notes
• Ask who to call for missing birth letter / proof-of-birth for leave claims
• Schedule first pediatrician visit per discharge instructions

Week 1 at home

• Create portal accounts for pediatric practice
• Add baby to health insurance by the plan deadline (often within 30 days — verify)
• Notify employer(s) of birth date for leave; submit proof when required
• Start / check short-term disability or state leave claim status if applicable
• File hospital and OB bills in one folder; do not ignore EOBs
• List every claim number and phone tree on one sheet

Birth certificate and Social Security

• Track birth certificate order status with your state / vital records office
• Order at least 2 certified copies if your state makes that easy (banks, benefits, passport later)
• Confirm SSN card mailing address is correct and secure
• When SSN arrives: store card offline; only share when required
• Note any correction process if name/date has an error — fix early

Insurance and medical admin

• Verify baby appears as covered member (call if portal lags)
• Set primary care provider on the plan if the insurer requires it
• Keep a running list of vaccines / dates from well visits
• Ask pediatric office how they handle prior auth / specialty referrals
• If NICU or extended stay: ask social work about additional forms before discharge

Employer and benefits

• Submit proof of birth for parental leave pay
• Confirm health premium payment method during leave (payroll vs invoice)
• Update W-4 / state withholding only if you choose to — consider a tax pro for bigger changes
• Add baby to employer life insurance / beneficiary designations if desired
• Dependent care FSA: check enrollment windows for new child (plan-specific)

Optional but time-sensitive

• Passport application if travel is planned in the first year (photos, forms, certified birth certificate)
• Will / guardianship update with baby's name
• 529 or savings account: open only when bandwidth allows; no rush required for paperwork validity
• Notify any union / professional board if leave status must be reported
• Daycare: submit deposit and immunization record when a slot firms up

Close-the-loop (end of month 1)

• Every open claim has a next check-in date on the calendar
• Certified birth certificate copies received or still tracked
• Insurance ID for baby in wallet / phone
• Pediatrician after-hours number saved as a contact
